It appears this should be made clear that this is designed to be for "Simple Health Communications + Online Workflows" in health services and that there will be a monthly fee after the trial period ends (listed as $49/ month).

The "Free" option is stated as being a "trial", which probably means after some unspecified period it won't work without paying them.
