Since my needs are not expansive I simply use Apple Notes for this and have created a special folder for my phone notes. I suspect one can use something like "TextExpander" to create a 'template' to paste into the note with a single keystroke.

There are two versions of this: the free PhoneLog, and PhoneLogPro, a bargain at $5.99. I popped for the paid version within two hours of trying out the free one. It’s a terrific app to corral information that might otherwise be on scraps of paper or electronic notes, or more likely, forgotten. Remember talking to a relative and not being able to remember the names of her grandkids, or what was the outcome of that health issue she had? Or calling a utility and wanting to cite chapter and verse of the previous complaints you’d made, and the corrective action they’d failed to take? This is the solution: a handy app to log conversations and messages that you exchange with family, friends, and vendors. There’s no learning curve whatsoever. Create multiple logs to suit your own purposes. It’s simple and foolproof. Equally important, the Pro version lets you drag in files and emails, so they become part of your reference material, without having to go search for them. It integrates with Address Book, and you can even set items as iCal events or iCal tasks for follow-up. In the past, I’ve tried to accomplish these things with Bento (unsatisfactory), Contactizer Express (now abandonware), or even putting stuff in the Notes section of Address Book entries, where neither text formatting or even spell-checking is supported. . The dev, who’s busily working on an iPad version, is very responsive and dedicated.
