The Aqua Extreme theme improves Aqua's appearance by refining parts of the GUI. These are the changes over regular Aqua:

Metal style from iTunes 6 applied system wide.
Improves Titlebar buttons.
Option to make titlebar buttons Blue.
Improves scrollbars to a more crisp Aqua style.
Improves progressbars to a more crisp Aqua style.
Improves small and large Round Buttons to a more crisp Aqua style.
Improves bevel buttons to a more crisp Aqua style.
Improves Small Push buttons (like in the Finder copy dialog) to a more crisp Aqua style.
