When nothing else works, I turn to MPEG Streamclip to get the job done. Very reliable and solid. This is just one of those indispensable pieces of software every Mac user should have. Thank you for keeping it free - the community loves you for it. :)
This new version is a considerable improvement over an already invaluable tool. It's faster and more reliable (I was able to transcode videos that crashed the older version).
The only problem I've found is quite annoying, though: when re-encoding an mp4 movie that refused to play on my AppleTV (instead of using the preset when encoding the DVD, I tried to make it higher quality. Serves me right), it did a great job, but doubled the length of the movie, adding blank space to the end. My 3-hour movie shows up as a 6-hour movie. It did not increase the file size. This has happened to 3 movies now - always double the original. Note: only when re-encoding an mp4.
This is a fantastic ripper, and works flawlessly most of the time (some of the newest movies introduce new DRM mechanisms that fool RipIt, but the developer updates the software very quickly). A recent update broke RipIt on my G5 Mac, but after an email to the developer, it was fixed and a new release posted within the day - that is amazing.
The software does a great job of making a perfect duplicate, free of its protection schemes, which the user can then use to backup their movie, or run through Handbrake to convert to an AppleTV or iPod format.
