The Facebook Top Friends extension is based on a javascript bookmarklet that Jeremy Keeshin recently released. You can read all the details on his blog, but essentially Facebook uses an algorithm of some sort to quickly preload links to users whom they believe you are most likely to search for on facebook.com. Facebook Top Friends merely uses Keeshin's code to pull up said list and gives you a nice drop-down menu of your "Top" friends.
It is important to understand that since this is merely the list that Facebook provides, these "Top" friends may or may not be what you are

It is important to understand that since this is merely the list that Facebook provides, these "Top" friends may or may not be what you are looking for. I found in my experience that this list is right on, and I know several other people have reported similar results. Try it out, and if you find it to be not useful, then it is highly possible that you are an enigma that Facebook can't figure out.
